We do only client or physician billing. We do not bill individuals or insurance. Please contact our client services to set up an account prior to sending specimens and test requests.
A completed requisition form must accompany all specimens. Please see the current price list before making your selections. Verbal requests must be followed by appropriate written documentation, either mailed or faxed to our client services.
Requests to add tests to existing orders:
A requisition or a Fax request must be sent for additional test requests.

Specimen Collection Guidelines
The quality of laboratory results is contingent upon the quality of the specimens, completeness, and accuracy with which tests are ordered. Proper specimen procurement, transport, and processing are essential to obtaining valid, timely laboratory results. All requisitions and specimens received for clinical testing must meet defined criteria for identification, collection, volume, and testing in order to be processed.
Specimens should be labeled at the time of collection. The information on the label should be checked again to ensure the patient's or donor's demographics agree with those on the requisition. The date and time of collection should be noted on the requisition.

Specimens can only be received during normal hours of operation, which is Monday through Friday from 0800 to 1700.
